By the Bay Health, an affiliate of UCSF Health, provides the highest quality hospice and palliative care, as well as skilled home health care services to patients and families throughout the Bay Area, including the most vulnerable patient populations.
Our affiliation connects UCSF patients to the largest independent not-for-profit network of hospice providers in Northern California, formed through the merger of three regional leaders: By the Bay Health, the oldest hospice in California; Mission Hospice & Home Care, the first not-for-profit hospice in San Mateo County; and Hope Hospice, one of the first not-for-profit hospices to serve the East Bay's Tri-Valley region.
The network serves patients with serious illnesses as well as those needing end-of-life care. Its comprehensive services include hospice care, home health care, palliative care, transitional care, care for children with chronic and serious illnesses, bereavement counseling and dementia support, as well as the only hospice residence on the Peninsula, Mission House. Patients have access to a compassionate team of doctors, nurses, social workers, spiritual support and bereavement counselors, home health aides, physical and occupational therapists, and other specialists.
