UCSF's Patient Capacity Management Center (PCMC) opened in early 2023 with a mission to transform the patient, provider and referral experience. PCMC uses advanced data analysis to improve communication and care coordination, aiming to streamline the patient experience from point of entry to discharge. Our goal is to ensure that every patient is receiving the right care in the right place at the right time.
Along with improving care for established patients, our staff works directly with referring hospitals to facilitate timely access when their patients need care at UCSF. We deeply value our relationships with referring hospitals and are committed to working collaboratively to serve the needs of patients throughout Northern California and surrounding regions.
How PCMC works
At PCMC, various UCSF teams – such as centralized patient placement, the transfer center, financial counseling, hospitality, transportation, and radiology – work together in real time. This system streamlines communication and problem-solving to manage hospital capacity and workflows.
Our capacity management center features interactive dashboards that centralize key data and predictive trends on hospital capacity, allowing us to optimize operational decision-making and care coordination across departments. We use smart tools and integrated systems to bring patients and their families the smoothest possible journey through our health system.
PCMC's impact
Since launching, PCMC has increased UCSF's capacity to support additional patients and reduce wait times, improving both care access and efficiency across the health system.
Overall, PCMC's data-driven approach has led to better patient experiences and outcomes.
