Request an Appointment: Physical Therapy at Mount Zion | UCSF Health

To request an appointment, please complete the following steps:

1. Obtain a referral for physical therapy or rehabilitation services from a provider at UCSF or a UCSF affiliate. The date on your referral should be within the past two months.
2. Confirm that your insurance covers physical therapy or rehabilitation services at UCSF. If prior authorization is required, request approval for these services from your insurance plan.

If your plan doesn't cover physical therapy and rehabilitation services at UCSF, you may be able to access this care in your community at an in-network practice.
