Request an Appointment: Physical Therapy at Mount Zion | UCSF Health

If you'd like an appointment, please complete the following steps:

  1. Obtain a referral for physical therapy, occupational therapy or speech-language pathology from a provider at UCSF or a UCSF affiliate. The date on your referral should be within the past two months.
  2. Confirm that your insurance plan covers rehabilitation services at UCSF. If prior authorization is required, request approval for these services from your insurance company.
  3. Call us at (415) 353-7598.

If your plan doesn't cover rehabilitation at UCSF, you may be able to access this care at an in-network practice in your community.
