The Care at Home Program provides home-based primary care services and longitudinal palliative care to homebound adults living in San Francisco. This program was established in 1999 with a philanthropic gift from the Richard and Maria Ury Endowment in honor of Dr. Harry Weinstein to teach medical students about home care for frail adults.

We care for patients age 65 and older who live in San Francisco and can't leave home due to illness or injury. We review requests for patients under 65 on an individual basis. Because UCSF is a teaching institution, we also teach students, residents and fellows.
Who We Are
Our team features doctors and nurse practitioners with expertise in geriatrics and palliative medicine. We make home visits, support patients and families with complex care needs, focus on understanding their goals and preferences for medical care, specialize in symptom management, and provide individualized care. Additional team members include registered nurses, social workers and health care coordinators.
Our team is specially trained to manage common illnesses and conditions in aging such as dementia (Alzheimer's, vascular and others), arthritis, and heart problems as well as frailty and falls.

Insurance, Funding and Referrals
Care at Home accepts these insurance plans. Before accepting a referral, our team will need to verify insurance authorization.
Our services are funded primarily through the generosity of private foundations and individual donors.
Referrals come from UCSF hospitals and clinics, community doctors, home care agencies, family members and adult protective services. Care at Home has 501(c)(3) nonprofit status through the UCSF Foundation. Donations are accepted online.
