Follow these steps to refer a patient for prenatal care, preconception services, or our Multidisciplinary Approach to the Placenta Service (MAPS) Program.

For urgent (within three days) or emergent referrals, call the UCSF Access Center at (415) 353-1611.

1. Gather required documents

• Completed OB referral form

  Download referral form

• Authorization from insurance company, if needed
• Any relevant medical records or test results (such as records of previous births, ultrasound reports, echocardiogram results and prenatal laboratory test results)
